I’m happy to share that Wholehearted Healthcare, P.C. is “in-network” with most major medical insurance companies. If you’d like to find out if your insurance covers our care, please call the phone number on the back of your insurance card to see if “Eugena Foster” an/or “Wholehearted Healthcare, P.C.” is “in-network” with your insurance plan.
If we are “out of network” with your insurance and you would still like to be a client, then Gena would be happy to see you. See the list of self-pay fees provided below.
“Shared” insurance plans are treated as “self- pay” and like with self-pay services you will receive an itemized receipt for your services. Then you yourself can submit your claims directly to be reimbursed for your out of pocket cost.
Many services can be paid with an HSA or FSA, pre-taxed for your convenience.
Lab work ordered off-site will be submitted to your insurance via the lab company where testing is completed.
Self-pay fees are as follows. All fees are due on the date of service.
- Initial New Client appointment: $275
- Yearly Physicals/Problems/Follow Up 31-60 minute appointment: $170
- Problems/Follow up 15-30 minute appointment: $100
- BioTE Natural Hormone Replacement: female pellet insertion $350, male pellet insertion $750.
- Vaginal/ Vulvar PRP: $700.
- Methylcobalamin (B12) injections $20.
